Who it is for

NHS Continuing Healthcare (CHC) is arranged and funded by the NHS for adults whose primary need is a health need. If eligible, the NHS funds the whole package — at home or in a care home.

How it is assessed

Assessment normally begins with a checklist, which can lead to a full multidisciplinary assessment. Ask for the outcome and reasoning in writing, and ask about the review and appeal route if you disagree.

Keep your own record

Keep a simple diary of needs, incidents and professional visits. Families who can show a consistent record find assessments far less stressful.
